    DISCLOSURE TO APPLICANT / EMPLOYEE THAT A CONSUMER REPORT – DRIVER'S LICENSE ABSTRACT MAY BE OBTAINED BY EMPLOYER


    In compliance with Public Law 91-508 (the Fair Credit Reporting Act), as amended by Public Law 104-228 (the Consumer Credit Reform Act of 1996) and applicable state law, this notice informs you that a Driver's License Abstract Report / Consumer Report may be obtained in connection with your application for employment or ongoing employment.


    "Consumer Reports" include but are not limited to credit reports, criminal background checks and Motor Vehicle / Driver's License Abstract Reports.

    • If hired or currently employed, I understand that my authorization shall remain valid during my employment unless cancelled by me in writing.

    • By signing this document, I authorize COMMAND INTERNATIONAL SECURITY SERVICES INC. and its insurers, brokers and vendors to obtain my Driver's License Abstract Report.

    • I agree to release and hold harmless COMMAND INTERNATIONAL SECURITY SERVICES INC. and its insurers, brokers and vendors from any claims relating to information contained in my Driver's License Abstract Report.
